Welcome to the Lockland Local School District Enrollment Center. Our enrollment staff will help make the enrollment process as seamless as possible for you. Enrolling your child starts with a number of forms to be completed before the actual student placement occurs.  Below is a list of items that are needed to complete the  process:

• Completed Enrollment through FinalForms – required

• Parent / Guardian Current Photo ID – required

• Name and address of previous school – required
  (phone number & Fax number is helpful is you have it)

• Birth Certificate – required
  students enrolling in an Ohio school for the first time MUST present an original birth certificate or acceptable alternative (acceptable alternatives include: a passport or attested transcript of a passport filed with a registrar of passports at a point of entry of the United States showing the date and place of birth of the child; an attested transcript of the certificate of birth; an attested transcript of the certificate of baptism or other religious record showing the date and place of birth of the child; an attested transcript of a hospital record showing the date and place of birth of the child; or a birth affidavit)

• Proof of Custody – required
  proof of marriage, court custody papers, or court filing

• Proof of Residency – required
  current mortgage, tax bill, residential lease, OR notarized residency affidavit with owner’s proof and parent/guardian business mail (utility bill, bank statement, insurance, or medical bill mailed within past 30 days)

• Health / Immunization Records
  Students living in United States less than 5 years must get

• FERPA Policy

A request for records for the last school your child attended is required. Information requested from your child's last school will include: attendance history, discipline history, grade/credit information, test scores and an Individualized Education Program (IEP) and Evaluation Team Report (ETR) if applicable. This information will assist us with proper placement of your child within one of our schools and/or programs. All applicable IDEA and Ohio Operating Standards will be followed to ensure appropriate services are provided to your child.
